Registration is Limited - Deadline: August 30, 2007

Registration with payment must be received by deadline in order for you to receive and study the manual to prepare for the course. The provider manual will be mailed with course materials and schedule.

Private hotel rooms are included with Option I and are arranged by the AZ BTLS staff. Hotel check-in is after 3:00 pm on Sept. 23 and check out by 11:00 am Sept. 26...or Sept. 27 for PBTLS registrants. Course lectures to be held in the spacious and beautiful Starview Room!

Substitutions of attendees are allowed but notification is required. Registrations received after 9/7/07--add a $25.00 late fee.

Option I

Option I: $ 325.00 BTLS Provider Course

· 3 Reserved nights lodging at The Riverside Hotel & Casino (Sept. 23, 24, 25) is arranged for you.

· BTLS manual, curriculum, handouts and a continental breakfast each morning.

· 16 hours CEU's.

· BTLS on the River T-Shirt!

· State and National BTLS/ITLS certification (upon successful completion of course.)

Option IP: $495.00 BTLS and Pediatric BTLS

· 1 additional night lodging at The Riverside Hotel (Sept. 26).

· Pediatric BTLS manual, curriculum, handouts, breakfast and lunch.

· 8 hours CEU's.

· State and National Pediatric BTLS (PBTLS) Certification (upon successful completion of course).

Option II

Option II: $275.00 BTLS Provider Course

For local providers, this option does not include hotel room, but still includes T-Shirt!

Option IIP: $450.00 BTLS and Pediatric BTLS
